MAKEUP APPLICATION CONSENT FORM
THE CONTRACT
We can travel for $150.00 for A standard makeup application.
We apply the makeup if only you purchase it.
If you buy the makeup THEN we can apply the makeup for free of charge.
We do not just do makeup for free or go into production.
After you purchase everything you like or need for your application we can apply it FOR that day ONLY For free .
For continued use of our service on a Daily BASIS WE WILL CHARGE BY THE HOUR. For the first hour 150 after the first hour we will charge 100 for each additional hour. For makeup retouches. For more than 8 hours we will start on the 8th hour at 200 and every hour after that will be 150
If you pay for a long distance travel round trip it must be paid in advance. booking and all fees MUST BE paid in advance. Across the country????? We will travel to you for you and only you. If you have other people they must be paid in before any application for them can proceed. If you did not know they would need an application then they must pay 200.00 before their process starts. Then we can all proceed. Each person then is 200 per application. Per hour. Please understand 1 hour is 200.00 per person. And every additional hour is 150. (They must sign their own contract.) They can sign or attesting to it by email is fine. THEY WILL HAVE THEIR OWN UNDERSTANDING OF THE PAYMENT PROCESS THEIR ADDITIONAL HOURLY FEES WOULD APPLY AS. Ask what discounts are available for more than 1 person. How can you save $50 per person.
In ANY case we will only accept payments in cash. We do not accept checks. Or any other form of payment for this at the moment.

We are A Certified Makeup Artist, Licensed Cosmetologist, and Licensed Esthetician and Certified Laser Technician for Skin, Body, Hair and Tattoo Removal, Certified in Hair Extensions keratin bonding and tape and we are Certified in CLASSIC, Volume, and mega volume Eyelash Extensions, Microdermabrsion, Microneedling, bb glow Microneedling Brow lamination, lash lift, brow tint and lash tint, henna brows, chemical peels, dermaplanning, skin tags, and dark spot removal.

By signing this contract you are agreeing to getting this service done. This is for any service. We are not responsible for any allergic reactions or any type of unknown infections that you believe that was causeD by this SERVICE in any form from our makeup or from your own personal makeup. This is because we do not know the state of your skin before this process nor do we know the state of your skin after this process. We take before and after pictures for the portfolio so we have a record of our work. If you sign this contract you are giving us permission to do your makeup or any other procedure dealing with eyes, skin, hair, or nails.

We want you to be happy as our client. By signing this you understand that things can happen so In the event of any skin mishap please agree that you will COME TO US FIRST AND LET US RECOMMEND THAT YOU go to your physician if you or we feel it is necessary. We always will suggest that to you because we don't know how your skin is BEHAVING before or after your service. We need you to continue to take care of your skin as you normally would, Or find another alternative to attend to your skin. If you have any cuts or bruises on your skin prior to getting the application you may get an irritation if you do not take care of your skin.
Please continue to clean your brushes and utensils as you normally would. Any and all items you use often carry buildup. It is suggested for makeup often more than less that you clean and sanitize them for or between clients. This is because makeup can irritate the skin if you're not protecting your skin by keeping it clean.

We only come with the utensils needed for the standard appointment as discussed prior to the appointment any other procedure being done we cannot be held reliable for so we may not be able to do that additional services. But you can pay for another procedure and we can come back. This is becaUse we may not have the required materials necessary because of no advanced notice.

This contract may change at any time in the future as new procedures, services, and processes are implemented often we kindly ask that you bare with us as we are getting other procedures, services and processes added to our list for you.

But the main policy is the same: no returns no exchanges AND all sales ARE final.
We supply, sell, and use discount makeup for any and or all application processes. Our makeup is final sale and is not returnable in the stores please take a look at the makeup you desire and be certain that you are satisfied with your color once you purchase it we cannot change it or accept it back. FOR SANITARY PURPOSES.

MINIMUM REQUIREMENT FOR TRAVELING IS $100.
